Dental & Medical Insurance benefit plans

We are participating providers for most dental benefit plans.  As a courtesy, we are happy to work with your carrier(s), both dental & medical, to maximize your benefits & bill them directly for reimbursement of your procedure.  We will gather details regarding your plan(s) at the time of your initial phone call.  Prior to your arrival in our office, we will contact your dental & medical benefit providers to gather specifics regarding your plan(s) & potential coverage.  Once our doctors have performed an examination & recommended a treatment, we will calculate an estimate based on the information we have gathered from your dental benefit provider.

We are a participating provider for the following dental insurance carriers:

• AETNA PPO
• AMERITAS PPO NETWORK This PPO network is used with other insurance companies such as: Principal. (please look for the AMERITAS logo on the patient benefit card)
• ANTHEM PPO, including ANTHEM FEP

• CONNECTION DENTAL NETWORK (works with many different insurance companies such as: United Health Care/UHC, GEHA, Great West, Coventry Dental, Prudental, UMR (please look for the CONNECTION DENTAL logo on the patient benefit card)
• DELTA PPO, PREMIER, EOP and TRICARE RETIREE
• DENTALGUARD NETWORK (Guardian)
• DENTEMAX PPO NETWORK (works with many different insurance companies such as: Medical Mutual, Meritain, UMR, EBA (please look for the DENTEMAX logo on the patient benefit card)
• DHA NETWORK Dental Health Alliance (Works with: Assurant, Nippon, CBC A)
• MAVEREST PPO NETWORK (Works with many different insurance companies such as: Humana, Coalition America/Stratose (please look for the MAVEREST logo on the patient benefit card)
• METLIFE PPO
• SUPERIOR DENTAL
• UNITED CONCORDIA PPO (National Fee For Service and Concordia Access)

Please note:

o We are out-of-network (a non-participating provider) for all medical plans.

o  We are a DMO/HMO/MANAGED CARE provider at the Bexley location only.

o  We are NOT a MEDICARE/MEDICAID provider  

o  We are NOT a Molina or CARESOURCE provider
