YA woman smilingour initial appointment will consist of a consultation & examination by the doctor.  We will explain your diagnosis & discuss treatment options.   Most patients leave that day with a treatment estimate and surgery appointment.  Occasionally, surgery can be performed the same day as the consultation; however, a complex medical history or treatment plan will likely require further evaluation &/or treatment scheduled for another day.

Please bring the following items to your first appointment:

  • REFERRAL SLIP from your referring doctor.  Your referring doctor may also submit an electronic referral directly from this website.

  • In some instances, recent X-RAYS taken by your referring doctor may work for our diagnostic purposes.  We encourage you to bring a copy with you  or ask your referring doctor to send them to us via e-mail ([email protected]).
  • A complete list of all MEDICATIONS & SUPPLEMENTS you are currently taking
  • If you have MEDICAL &/OR DENTAL BENEFITS, please bring the cards with you.  (Some benefit plans do not provide cards.)  Even though we gather most of this information during your initial phone call, we’ve found that having copies of your cards helps with benefit processing.
  • PHOTO ID

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation visit.

WHAT DOES THE PROCEDURE COST AND IS IT COVERED BY INSURANCE?

The fee for your treatment is determined by a number of factors. These may include the difficulty involved in removing your teeth and which type of anesthesia is best for you. During your consultation appointment, the surgeon will need to review your x-rays, complete an examination and determine the best option for anesthesia before an accurate estimate can be provided. Every insurance company has a different policy regarding the extent of coverage for a given surgical procedure. We will help you obtain maximum insurance coverage for your treatment.
